SAM FAQ  Updated – May 1st 2020 

1.      I’ve updated the Starter Kit Checklist and HS Checklist, but some checklist items don’t have an upload folder?

The new Starter Kit and HS checklists have the correct line items (so when a brand new family gets the HS checklist, they will see the correct line items), but for some reason when we update the checklists we are getting some of the old checklist items too.

·       Statement of Risk is an old checklist item, Inter-Country Adoption Risk Statement is the replaced checklist item and has an upload folder. Use that folder.

·       ICAT has been eliminated from the Starter Kit all together and is not on the new Starter Kit checklist so there is no folder, don’t worry about uploading.

·       Intake Principles has been eliminated from the Starter Kit all together and is not on the new Starter Kit checklist so there is no folder, don’t worry about uploading.

·       Post Adoption Worksheet is on old Starter Kit checklist, but is listed together on new HS checklist (we have moved both Post Worksheet I and II to be in the main program manual).  There is a checklist item to upload these in the HS checklist. 

 

2.       Help!  I updated the Starter Kit and HS checklist and all of my previous dates erased.

 

We had one instance (that I know of) where the worker added the new HS checklist and the previous dates entered were erased.  I am hoping this is just a glitch. To be safe, it may be best for you to take a quick screen shot of the checklist before updating it so you have a list of all the dates and don’t have to go digging for them in case they erase.

 

3.       I’m trying to add documents to the Contract International HS checklist and there are not upload folders.

 

We are treating Contract HS like another program, so the checklists have not be updated with upload folders.  We plan to add Contract to the portal late June.

 

4.       Who do I send a family to if they have a question on the Portal that I cannot answer?

 

At the top of the Family Portal, there is a “Support” button.  They need to click on that and will directed to Josiah’s email. Josiah.moore@lifelinechild.org 

 

5.       If I see something that needs to be edited in SAM, what do I do?

 

Please send your questions to your CWS or Program Director/Manager. They will compile a list and email them to Josh and Josiah (during Marcy’s maternity leave).

If you encounter something that is broken and needs an urgent solution, please email Josiah and copy Josh. 

 

6.       What do we do if a item on the checklist doesn’t apply to a family?

 

Deleting a Checklist Item

a.       OPTION #1:

                                                               i.      In the right hand column at the top of the Case Checklist page, select “Advanced Options”

                                                             ii.      Select “X Remove Items”

                                                           iii.      The checklist page should then re-populate with a red next to each checklist item

                                                           iv.      Click the to remove the checklist item

                                                             v.      Back in the top right corner of the page, select “Default View” to see the restored checklist view

b.       OPTION #2:

                                                               i.      Click on the gear/wheel on the far right side of a particular checklist item

                                                             ii.      That will bring you to a new page “Edit Checklist Item”

                                                           iii.      Scroll all the way down to the bottom of the page and click on “Delete this Checklist Item”—it is in the bottom right corner and very faint

 

7.       I am trying to upload documents for a HS Update checklist, but there are no upload folders.

Upload folders have been added to the HS Update checklist now.

 

8. How do I add case notes to a file 

  1. EMAILS:
    • Go to the family’s “Communications” page
    • Select “+ Add Family Communications” in right corner 
    • Fill in Date/Time, Communication Type, Subject/Communication Topic/Topics Covered
      • If the family has adopted more than one child and this case note is about a specific child, be sure to specify the child in the subject line
      • You can select as many of the topics covered as you need
    • Copy and paste the body of the email into the “Body” section
    • Click Save
  2. PHONE CALLS:
    • Go to the family’s “Communications” page
    • Select “+ Add Family Communications” in right corner 
    • Fill in Date/Time, Communication Type, Subject/Communication Topic/Topics Covered
      • If the family has adopted more than one child and this case note is about a specific child, be sure to specify the child in the subject line
    • Bullet point the phone call in the body of the case note
      • I would encourage you to have the SAM case note page up as you are on the phone with the family
    • Click Save
